What you need to get a funeral cover claim processed

Certified copies of the following documents are required:

  • Completed EcoSure Death Claim Form
  • 2 IDs of witnesses
  • Burial Order or Death Certificate
  • Claimant's Bank Account (Current Account)

In the event of death of the insured, the Claimant or Beneficiary must visit the nearest approved EcoSure Agent, Funeral Service Provider, or email the necessary documents to claims@cassavasmartech.co.zw as soon as possible from the date of the Insured's death. The claims can also be submitted via WhatsApp on 0777 222 222 or on the website at www.ecosure.co.zw/claims. All claims must be made no later than 180 days from date of death.

 

How to claim for Personal Accident Cover

After an accident that has caused disability the following documents are required:

  • Medical report from at least two qualified medical doctors
  • Completed claim form
  • ID of the policyholder
  • ID of claimant (if different from the policyholder)
  • A police report of the accident
  • Claimant'sBank Account (Current Account)

After an accident that has caused accidental death the following documents are required:

  • Death Certificate, Burial Order or Order to Bury or Affidavit from the Chief confirming the death of the deceased.
  • ID copies of Claimant and two (2) witnesses with at least two bearing the same surname as that of the deceased.
  • A police report of the accident
  • Claimant's Bank Account (Current Account)

How to claim for Accidental Death (Personal Accident Cover) 

After an accident that has caused accidental death the following documents are required:

  • Death Certificate, Burial Order or Order to Bury or Affidavit from the Chief confirming the death of the deceased.
  • ID copies of Claimant and two (2) witnesses with at least two bearing the same surname as that of the deceased.
  • A police report of the accident

  • Claimant's Bank Account (Current Account)

How to claim for Hospital Cash Back Plan

To make a claim the following documents must be submitted:

  • Fully completed claim form
  • Patient card
  • Other medical records from the hospital
  • Medical aid card used
  • Receipts for all cash payments
  • Letter of admission and discharge
  • Copy of ID of claimant.
  • Claimant's Bank Account (Current Account)

The holder of the cash-plan or the next of kin of the principal can submit a claim if the policy holder passes on after being hospitalised. In addition to afore-mentioned documents, a death certificate, ID’s of 2 witnesses and proof of relationship to the deceased.

 

How to claim for EcoSure Memorial Benefit

There are no documents required for Memorial Benefit claim. The benefit is paid 40 days after burial of the deceased. The benefit is paid directly to the appointed Beneficiary EcoCash wallet and no paperwork is required.
